If you are a patient, here is some good news! Patients/Clients are welcome to self-refer to the Chronic Condition Coaching Clinic (CCCC). NO referral is required to be seen in the CCCC.
In addition, any and all other health care professionals are most welcome to refer their patients/clients/customers to be seen in the Chronic Condition Coaching Clinic.
