Self-Management Support
James Cook University is offering people living with chronic conditions access to their Chronic Condition Coaching Clinic (CCCC). The clinic provides one-to-one support and coaching to assist people to optimally manage their chronic health condition(s).
Together with a Registered Nurse coach, you will:
- Define your health related goals and aspirations.
- Develop your skills and confidence in effective self-management of your chronic conditions(s), in partnership with your health care providers.
- Move towards your own vision of optimal wellness and functioning.
We are open every Monday morning and meet with patients by appointment. The initial hour appointment is at a cost of $20 and follow up 30 minute appointments are $10. For further information, email Dr Kristin Wicking on ipclinic@jcu.edu.au.
